Dental Emergency: What to Do When a Permanent Tooth Gets Knocked Out

Life can surprise us at any moment — a fall during playtime, a sports injury, or an accidental bump can sometimes result in a knocked-out permanent tooth.
While this can feel frightening, the truth is: your actions in the first few minutes make all the difference in saving that tooth. As your dental care provider, I want to ensure you know exactly what to do in this kind of dental emergency.
Why Time Is Everything
When a permanent tooth is knocked out (also called avulsed), it’s considered a true dental emergency. The first 5 minutes are absolutely critical for the long-term survival of the tooth. Prompt action can mean the difference between saving and losing it.
What to Do Immediately
If this ever happens to you or someone in your care, here’s how to respond quickly and safely:
1. Locate the Tooth — and Handle It Carefully
Hold the tooth by the crown only (the white, chewing surface).
Do not touch the root. The root has vital fibers that help the tooth reattach.
2. Rinse Gently If Needed
If the tooth has dirt or debris, gently rinse it with water for no more than 10 seconds.
Do NOT scrub, brush, or dry the tooth.
3. Try to Reinsert the Tooth
If possible, place the tooth gently back into its socket in the mouth.
Have the person bite down softly on gauze or a clean cloth to keep it in place.
4. If Reinsertion Is Not Possible…
Store the tooth in a small container of cold milk or saline solution (not water!).
Alternatively, place the tooth inside the cheek pouch (only if the person is conscious and won’t swallow it).
Get to the dental office immediately.

If a tooth has been knocked out, it is essential to be seen by a dentist within 30 minutes — ideally no later than 1 hour. Even if replantation isn't possible right away, we can assess your needs and prevent further complications.
For severe trauma involving permanent teeth, even if the tooth has been put back in place, a professional evaluation is needed within 6 to 12 hours to give the best chance for long-term health.
